If you patch to Blight, you will get the 384.71 client. Live had not been updated yet.

Sorry I didn't answer your question directly.
Blight always has a newer version number than Order or Chaos (Live servers). This is because the developers are quick to push updates to Blight for testing and review.
In your example, it is very normal that Order and Chaos (Live servers) are at patch version 384.65.3 while Blight is at 384.71. The launcher will automatically patch you to the appropriate version.
If you are playing between Blight and Live often, there's a couple options:
1. Always patch to Blight and disable patching when connecting to Live (if you want to avoid always updating)
2. Have two installs of Istaria, one for Blight and one for Live (Order and Chaos)
